To be accepted for membership in CAP you must have a Social Security number. If you do not yet have one assigned to you, you will to obtain one before your membership can be completed.
THE FOLLOWING FORMS WILL NEED TO BE COMPLETED AND SUBMITTED TO THE MEMBERSHIP OFFICER.
The forms below must be printed (no cursive) or typed or printed in black ink. Do not sign the forms until they have been reviewed. Membership forms can be found at the GoCivilAirPatrol site for either Cadets or Senior Members.

- A copy of your latest school report card (with parent’s signature)
- A check made out as follows:
- cadet application: $38 made out to “Civil Air Patrol HQ”
- senior member application: $65 made out to “Civil Air Patrol HQ”
